When installing InConcert server you have the choice of installing a "New" server or an "Add Additional Server".

Symptoms:

Installation fails with the error message as described in the Title.

Cause:
=====
When additional server are installed one more line is added in the database table SERVER_NAMES for each additional server added.  The server name must be unique in this table hence the error message.

Resolution:
=========
Install the additional server with a new name.
