Resolution
1. Login to Data Entry Portal : http://[ServerName]/DataEntry/
2. Click on the sample that the history should be loaded for.
3. Click on "View Sample revision History" button to see the sample revision.
4. Optionally, click on csv to save this information in a csv file.
