This hotfix can be downloaded from the SFDC Customer Portal Web UI, using your username and password for support.tibco.com. Once logged on you can find the hotfix under Download Menu:

Closed Issues in 6.9.1_Hotfix 3(This Release)

DCRM-2045: Getting 'Number of concurrent requests exceeded the limit of 52' error for Dynamics 365 Online Server.

Note: 
DCRM 6.9.1 HF-003 helps the applications to optimise applications throughput by removing the affinity cookie in the WEB API requests to Microsoft Dynamics 365 Online server. But the hotfix do not implement any management of Service Protection API limits . 
The issue is already mentioned as part of Known Issues in Release Notes document and the corresponding workaround is provided to deal with Service Protection API limits.
