Resolution:
The Agent detects if the server Publisher exists by using RV's FT's passive monitoring. Make sure you configure the "Fault Tolerance Monitor Name" for the Agent to be the same as the "DQ Name" of "ServerPublisher_RVDQ" session for the Publisher. If the two names are different, the Agent cannot "see" the Publisher even though the Publisher instance is running.
