Resolution:
You need to start the Server Publisher if it has not been started already.
If the Server Publisher is indeed running already, then most likely the Agent and the Server Publisher are not sharing the same DQ group name. Please check the "DQ Name" parameter of the Server Publisher's RVDQ session whose name, by default, is "ServerPublisher_RVDQ". This name has to match the configuration for Agent's "Fault Tolerance Monitor/Name".
