Resolution:
One cause of this is having two or more InConcert servers with the same logical name for the InConcert server. The problem is two separate applications have claimed with RV that they should receive all messages with this subject. This causes conflict bring down the InConcert Servers.
Change the name of one of the servers in the config file and in database in table SERVER_NAMES.SERVER_NAME.
